Osijek-Baranja County Strengthens Support for Older Adults: ELEVATE Pilot Activities Set to Begin. Local Working Group agrees on concrete steps - from rural outreach to day care and a mobile community team

A stronger, more connected support network for older adults is taking shape in Đakovo area of Osijek-Baranja County. On Thursday, 27 February 2026, the Local Working Group met at the Home for elderly and infirm persons Đakovo as part of the Osijek-Baranja County project ELEVATE. The meeting confirmed a key message: when the right partners sit at the same table with a shared purpose, solutions become both realistic and measurable.

Representatives of Osijek-Baranja County, the Home for elderly and infirm persons Đakovo, local self-government units from the Đakovština area, health institution, educational and religious institutions, and civil society organizations took part in the discussion. The focus was clear - how to improve quality of life for older adults and strengthen community-based support, especially in rural areas where services are often harder to access.

At the heart of the meeting was the presentation of three pilot activities designed by expert goup to bring tangible improvements on the ground:

·         Raising awareness and providing information on services and activities for older adults across nine rural municipalities

·         Establishing a half-day day-care programme within the Home for elderly and infirm persons Đakovo

·         Forming a multidisciplinary mobile team to deliver community-based services directly in the field

The discussion pointed to strong readiness for joint action. Municipal representatives expressed willingness to provide space and local logistical support, educational institutions and associations announced the involvement of volunteers and Health Center OB County participation of visiting nurses in preventive examinations. Particular emphasis was placed on the importance of cross-sector cooperation, mapping real needs in local communities, and ensuring the long-term sustainability of activities beyond the project’s completion.

According to the project team, the pilot activities will begin in April 2026, marking the start of a practical, community-oriented model of support that could serve as an example for other areas as well.
